The Case Assessor should have:

  • A four-year Bachelor of Arts or Sciences, or RN degree, or equivalent four-year experience in geriatric case work or social work in community/social program(s)
  • Computer experience and word processing software skills required;
  • A reliable vehicle and valid driver’s license;
  • Knowledge of Onondaga County community services preferred;
  • Familiarity with eligibility criteria for all entitlement programs and experience working with seniors preferred.

Back up Office support should have:

  • Experience in customer service, administrative s, and/or call centers
  • proficiency with Microsoft Office, demonstrated ability to use client management software required; experience dealing with older adults a plus.

ABILITIES:

The Case Assessor should:

  • Be patient, caring and enjoy working with people who are homebound;
  • Be organized, thorough and follow through on details;
  • Handle information confidentially;
  • Have good time management skills;
  • Possess good oral and written communication skills.

Back up Office Support should have:

  • Strong time-management and communication skills, flexibility and ability to work with a wide-range of people, and multi-tasking capability required.

RESPONSIBILITIES:

The Case Assessor shall:

  • Conduct detailed intake and home visits for assigned County-funded grant recipients.
  • Schedule home visits, provide program education, and complete computer client assessments for new clients.
  • Transfer assessments to Onondaga County database (PeerPlace)
  • Visit and assess WIN clients within ten (10) working days of beginning service;
  • Schedule and conduct 6-month re-assessment for funded clients;
  • Conduct event-based assessment when necessary (after hospitalization);
  • Complete computer documentation and narratives on each client following

home/telephone contacts and transfer data to Onondaga County;

  • Act as an advocate for all clients, contacting and referring services, as needed
  • Assist clients when problems develop related to meal service;
  • Update physician’s diet orders once a year, or as needed;
  • Maintain various statistical reports as required by the agency;

The back up Office support shall:

  • Be responsible for Adminstrative duties associated with the front office and day to day operations of the business, when the Business Manager is not working

Meeting the Mission of the Agency:

  • Attend regularly scheduled outreach and in-service meetings of Onondaga County Department of Aging;
  • Assist staff and volunteers, where needed, to insure the success of the Meals on Wheels program.

Job Type: Part-time

Pay: $20.00 per hour
